Patient Rights & RESPONSIBILITIES
Patient Rights:
1.To receive high quality care appropriate to the patients needs and consistent with the scope of the hospital.
2.To receive considerate care regardless of race, religious , belief and age.
3.To Know the name of the physician who has the primary responsibility for co-ordinating care.
4.To receive information about the illness ,treatment and prognosis.
5. To receive counseling regarding the estimated cost of the treatment and payment schedule at the time of admission as well as subsequently.
6.To be provided with confidentially during examination or treatment.
7.To be informed and be asked if the patient wishes to participate in medical research when it is being conducted at the hospital.
8. The patient is assured of the confidential treatment of the medical records.
Patient Responsibilities :
1. To provide complete and accurate information including full name , address, telephone number, date of birth , insurance details whenever necessary.
2. To provide accurate and complete information concerning present complaints past medical history, hospitalization and other matters relating to the patient health.
3. To follow the treatment plan advised by the physician, including the instructions of nurses and other health Professionals as they carry out the physicians order.
4. To treat hospital staff and other patients with dignity and respect and not to conduct any activity that will disrupt the work of the hospital.
5. Not allowed to bring alcohol, unauthorized drugs and weapons into the hospital ever.
6. To respect that hospital is a non smoking zone .
7. To follow visiting hours of the hospital.
8. Leave valuable at house and bring only those items necessary during the hospital stay.
9. Assure that the financial obligation of the patient care are fulfilled as per hospital policy.
